Your Mission in Action

An Unaccompanied Refugee Minor (URM) Case Manager is responsible for providing case management and supportive services to children and youth in foster care and foster families, referred to the URM program. The position is to act as a liaison between the referring agency functioning effectively with moderate supervision following agency standards, state, and federal regulatory requirements. The mission of the Upbring Unaccompanied Refugee Minor (URM) Program is to promote effective resettlement and rapid self-sufficiency for refugees in the state.

Responsibilities

  • Provide case management services and support to foster care parents and families and URM in accordance with licensing standards, contract agreements, agency and CARF standards, and funding requirements 
  • Conduct monthly home visits for assigned caseload to assess URM’s mental, emotional, and social well-being. Additional visits may be required if applicable  
  • Create, update, and maintain accurate and up to date URM case files and records with information pertaining to child’s medical history, treatment plans, and observations; chart all pertinent information in the UC case files/records in accordance with Agency policy and licensing standards 
  • Complete case notes and ensure all case files and documentation are completed accurately and in a timely mannermaintain case logs for all assigned URM 
  • Provide on-call services on rotation and during period of high influx with URM admissions and sponsor/foster family needs 
  • Work closely with management and the team to ensure case management and program services are effectively and efficiently supporting Agency objectives and program purpose     
  • Work closely with service professionals, court officials, parents and families, clinical staff, and service providers to develop, manage, and evaluate URM needs 
  • Respond to URM and foster parents and families regarding needs and concerns with compassion and empathy in a timely manner 

Qualifications

Minimum Qualifications

  • Bachelor’s degree in Social Work, Human Services, or closely related field  
  • Three (3) years’ relevant experience  
  • Bilingual in English and Spanish (Based on service area and service population) 
  • Must be 21 years of age or older per licensing requirements 
  • Must be willing to work evenings and Saturdays as scheduled 
  • Ability to establish rapport, trust, and boundaries with children, parents, and families  
  • Strong interpersonal skills with the ability to interact with diverse groups of people 
  • The ability to apply compassion and empathy, while dealing with stressful and difficult situations   
  • The ability to work independently and collaboratively within a group 
  • Strong organizational and time management skills with the ability to manage heavy caseload and meet deadlines 
  • Proficient in Microsoft Word, Excel, PowerPoint, and Outlook 
  • Excellent written and verbal communication skills 

Preferred Qualifications

  • Master’s degree in Social Work, Human Services, or closely related field.  
  • Five (5) years’ relevant experience

Physical Demands & Work Conditions
  • This position requires sitting and looking and using a computer for long periods of time 
  • Position works in a child facility and has constant exposure to children 
  • Position requires frequent standing, walking and time in units/cottages which are areas where children reside 
  • Lift, push, pull, move up to 50 lbs.
