Your Mission in Action

The Intake Placement Coordinator is responsible for managing and completing the intake process for every youth who enters the TFC Programs. The intake process consists of completing all initial intake paperwork and transporting minors to foster homes. This position will not be responsible for a Case Management caseload. 

Responsibilities

  • Monitor ORR Intake emails and assess acceptance of minors into the program 
  • Review ORR Intake email information with foster parents 
  • Coordinate the placement of minors with third-party transport company 
  • Complete the initial intake process, including transporting minors to foster homes 
  • Complete Preliminary Service Plan (PSP) within 72 hours of UC placement 
  • Coordinate any identified and immediate needs for minors through community-based services 
  • Transport youths to and from the facility or foster home setting as required 
  • Review and complete the placement packet with foster parents and UC 
  • Work in collaboration with leadership and the People and Culture Team to arrange training; when needed, coordinate and/or provide in-service training for staff development 
  • Regularly review employee performance, including goals and progress, to provide developmental feedback: use applicable agency development tools to facilitate growth and development in staff 
  • Draft and issue the 90-day and annual evaluations, along with supporting documentation, as per established procedures; work in collaboration with staff to set goals during evaluation periods

Qualifications

Minimum Qualifications

  • Bachelor’s degree in social work, Human Services, or closely related fiel 
  • One (1) year of relevant experience 
  • Ability to establish rapport, trust, and boundaries with children, parents, and families 
  • Strong interpersonal skills with the ability to interact with diverse groups of people 
  • The ability to apply compassion and empathy while dealing with stressful and difficult situations 
  • The ability to work independently and collaboratively within a group 
  • Strong organizational and time management skills with the ability to manageheavy caseload and meet deadlines 
  • Proficient in Microsoft Word, Excel, PowerPoint, and Outlook 
  • Excellent written and verbal communication skills 

Preferred Qualifications

  • Master’s degree in social work, Human Services, orclosely related field 
  • Five (5) years of relevant experience

Physical Demands & Work Conditions
  • This position requires sitting, looking and using a computer for long periods of time 
  • Position works in a child facility and has constant exposure to children 
  • Position is a graveyard shift 
  • Lift, push, pull, move up to 25 lbs.  
  • May be required to apply safety intervention with youth that could weigh over 100 lbs.
