Your Mission in Action

The Clinic Medical Support Professional is responsible for providing South Plains area residents seeking non-emergency assistance with health screenings and basic health and wellness education, specializing in long-term diabetes care in accordance with Agency policies, licensing standards, and other federal, state, and local regulatory requirements. 

Responsibilities

  • Provide patients with routine health screenings, such as, but not limited to blood pressure checks, blood sugar tests via finger stick, and temperature checks
  • Provide patients with basic health and wellness education for patients with chronic health conditions
  • Provide health education to patients as it relates to promotion of proper health and wellness, nutrition disease prevention
  • Initiate patient treatment plan based on prescribed health care needs and goals, physician orders, and health assessments
  • Monitor and identify patient medical and behavioral changes and notify appropriate medical and/or behavioral health professional; document changes and responses using correct forms and documentation standards
  • Ability to drive to community partner sites and work with the public
  • Complete required documentation and record keeping based on licensing standards, Agency policies, and federal and state regulations
  • Document and update patient charts/files with accurate and complete
  • Work closely with Director and interdisciplinary team to provide the care that focuses on patients in need
  • Work as team player and exhibit professional behavior and a positive attitude with both internal and external clients that reflects positively on the Agency and is consistent with Agency policies and practices
  • Always develop and maintain positive working relationships with patients and interdisciplinary teams within established boundaries

Qualifications

Minimum Qualifications

  • Registered Nurse (RN) license in the state of Texas
  • 2 years’ direct patient care experience
  • Valid Driver’s License
  • Demonstrated ability to use clinical skills in a fast-paced environment with competing requests
  • Ability to work under pressure using solid and appropriate decision-making skills
  • Ability to work collaboratively with diverse groups of people
  • Strong time management and organizational skills with the ability to complete assigned tasks during shift
  • Strong interpersonal skills with the ability to effectively communicate with diverse people
  • Ability to establish a positive rapport with clients 

Preferred Qualifications

  • Bilingual in English and Spanish with the ability to fluently converse and write in Spanish (based on the population served)

Travel

  • This position may be required to travel up to 25%
